My be use a crate some prefer the enclosed space to make the feel safer similar to a cat. For the next two weeks work on training. Put him in the car, for about 30 seconds, then take him out and give him a treat like Cheerios. Then repeat this a few times. They doing this once in the morning and then again at night. Do this for a couple days. Gradually increase the amount of time just sitting in the car. Do not drive for the first week. Then after doing this for a week, put him in the car and drive down the street, turn around and give treat once out of the car. Do this a few times gradually increasing the drive. If you do this, be patient and go slow. He will associate car with treats and good things. This will give you an idea if it is fear or car sickness. When going to vet buy special treat. Feed small breakfast so pup is hungry for treats later. Drive to vet park a few stores down from vet if possible. Get him out of car and give small piece of special treat. Walk to the vet office (unless to young then carry him). When outside office door give him another piece of special treat. Walk inside office, give several pieces of special treat. Keep giving treat off and on while waiting. Once enter exam area give treat again. Keep giving treats during exam. Give treats when leaving office. Your goal is to associate vet with yummy treat. My treat sizes are about the size of my finger nail. They don't need big amounts just that taste. My teddy preferred boiled chicken. I made it ahead of time then put it in a baggy and carried it in lunch container with ice pack to keep fresh. My boy hated the vet but now he loves visiting her because it means special treats.
If he is still throwing up than it is motion sickness.
