Sophie So sorry to hear about her cataracts. The surgery can be expensive and require intense post op care, but is very successful. It is much more complex for a dog than a human and the chance of inflammation can be a risk even after the surgery. It cost me around $3,500 per eye, so I can imagine that is hard for a rescue org to come up with that kind of money.
I have heard of the supplement but only the vet would know if it would help in your case, and you said he was optimistic. Praying that is so. It was never given as an option to me. When my dog had cataracts the vet said one main risk was glaucoma, that can cause pain and if not treated, loss of eye. Also the eyes can get uveitis. As my dog was only 1.5 years old I decided to do the surgery. He is fine now. I was told that cataracts in dogs often goes along with Diabetes, but luckily my boy did not have that. It must have been genetic.
From my (non vet!!) experience it would seem that the 2 risks, glaucoma and uveitis, could be managed with eye drops and monitoring of eye pressures and eye health. Unless of course the supplement drops you mention do in fact take care of the cataracts.
