I would've been frantic had anything like that happened to any of my animals. Good on you for getting it sorted right away and that Zhoie is okay.
You don't need a waiver for the DHPP. As for rabies, it all depends upon the laws in your state. Some give waivers, some do not. If she were my pup, there is no way she would have any shots. Speak with your vet about it!
