I would go to a Vet Derm - they will get you squared away and on the right path of treatment. Once you have complicated skin issues that aren't going away, it's def time for a specialist.

You should also ask your vet about trying Atopica - it can be a miracle for allergy dogs. Btw, did the vet do a skin scrape of the skin and look at it under microscope? If not, I'd sure be seeking another vet.
