Welcome to Yorkie Talk. Has your vet tried an antibiotic for your little guy? I would also recommend a senior blood panel if you haven't done one recently.
Dogs and people with weakened immune systems or diabetes are prone to skin staph infections.
My 4 year old had a few black crusty spots on his belly last year. He had a couple rounds of antibiotics that cleared them up, but I didn't want to keep using antibiotics every time he had one. We started using organic coconut oil on them (Cocotherapy or Nutiva are my favorite brands), and added a fish oil supplement to his diet, and he hasn't had a spot since.
