Video it and take him for an exam. Talking doesn't get testing done. If that vet seems to not know or isn't taking your serioously, go to a small-dog vet - one who knows Yorkies. Sounds neurological but it could be something like hypoglycemia causing this or something like that. But the dog needs to be seem by someone who will test and look for something going on. Walking sideways and disorientation are not typical. Pups of 4 months age usually wake up ready to go and full-speed ahead.
