Medical Pet ID Tag - Need Medicine Pet ID Tag - Free Engraving Lifetime Guarantee

I want to order this for Taycie. I will have her wear it while we are out and about. I like the idea of a Medical Tag because I would hope if something awful would happen this would help get her returned if they knew what she entails.

So... what would you ladies have engraved? Obviously my phone number of course, the front obviously says needs medication, so I am wondering if I should have that engraved?

I was thinking something like (and these are just thoughts at the moment):
Special Diet
Liver Diease
Needs Meds

This particular Tag can have 20 characters.. I think including spaces.
I obviously can't write her entire novel on it so I am trying to keep it brief and to the point, so if something did happen it would get her home.

What would you want to see engraved?
