My how I feel for you, your patience must be wearing thin. I do not know what you can do about the crate peeing as it seems you are doing EVERYTHING! But perhaps when he is out and about with you, you could tether him to you so he cannot do anything without your knowledge?
Although young pups pee a lot and have little control yet, is it at all possible he has a bladder problem?? A friend had the same issue with her puppy and found the pup had (irritating) crystals. Hope it is just youthful behavior and that there is no health reason.
