Do you crate train? Puppies lack full bladder control until about 6 months. Until then it's their age in months + 1. So a 4 month old should be able to hold it's bladder for 4 to 5 hours. Every dog, including puppies, needs to go to bathroom first thing in the morning, just as people do. So the best way to find his tell is to take him out of his crate in the morning and put his leash on. Stand in front of the door for 30 seconds, if he doesn't do anything try again the next day till he does. He should do something such as whine, bark, scratch you or the door, or act different from normal in anyway. That will be your tell. This is coming from a Certified Pet Trainer.
