Puppies do not need to be tested but adult dogs do. When I started my adult dogs on heartworm 8years ago, I did nit know about the medication. They tested my dogs and I am glad they did because my lab ended up having heartworm. She got from our visit to the mountains. If she got the meds first she would have died from blockage. We had to treat the worms first. 3 months crated while taking medication was a nightmare for a young active lab who did not know she was sick.
