I just wondered about a Yorkie going without water for 24 hours before it needs veterinary attention. I'd wondered if it shouldn't read "refused to drink water for 12 - 18 hours unless advised by the veterinarian". I also wish in addition to "injuries", "eye discoloration, swelling, prolonged squinting" could be added to #5 on the list. I've seen a dog out walking with such a red eye, swollen, very ugly and the owner says "well, I know he didn't hurt it - he's just allergic or something I think - we'll see". If they see the word "injury" alone, a lot think a stick to the eye or something they saw happen - something obvious.
Like Nancy suggested, I would like to add "any human/other medicine not currently prescribed for the dog".
Not being a vet though, I'm extremely hesitant to change anything. Maybe Ellie May & some others can add some ideas or maybe it is really okay as it is but I think if we don't keep it fairly short and sweet, it will get only partially read.
