Teething does that, the mixture of blood, saliva, and rotting food caught in the crevices is not pleasant. But since you mention a fishy smell, you might want to make sure, it's not coming from the opposite end, the anal glands. I think introducing them to the toothbrush early is important, even if they lose their first teeth.
