Default Help! My toy Yorkie has bald spots.

My sweet baby Simba has a bald spot on his back about 2 inches above his tail. It is about 2 inches wide and 4 inches long. It is almost an oval shape. I have had him to our vet twice for this problem. There are some little pimples almost resembling black heads in a small area. Now I've notice he has two more little bald spots, also oval shaped but no pimples and much smaller. At 2 separate times the vet has done scrapings and has found nothing. She seems to think that it is some type of acne and suggested using a Clearasil Benzoyl Peroxide Scrub and also a benzoyl peroxide, which is very drying to his skin. I use it on him about twice a week. She has o other ideas. I have changed his diet to grain free hoping that it may be an allergy. He has been grain free for two weeks now and so far I really don't see a difference. Fortunately Simba loves the grain free dry food, biscuits and treats but not the grain free wet food. I think I will continue with the grain free of another 2 weeks. If I see no results I don't know what to do next. If anyone has any suggestions, please HELP! Where do I go from here? A veterinary dermatologist?
