Pad holders tend to cause problems, from what I have read. Try putting the pad on a slightly larger piece of linoleum or rubber piece, nothing with raised edges. This will help establish the "potty area" and distinguish it from the carpet.

Great job with the praise and rewards, and cleaning the accident spots. Limit his freedom to a relatively confined area with the pad. Make it an area that you can supervise him. When he is alone, make the area really small, like a room or even an xpen. Do this until his habits improve and then slowly increase his freedom.

Choose one or two places for the pad. Don't move the pads around the house. Make sure they are always convenient. If he is a little guy, make sure he doesn't have far to travel.

Go nuts with the praise and make a big deal out of picking up the used pad. My boys seem to like that part and want the #2 picked up right away.

Another thing that might help is a spray scent attractant on the pad. I know there are many advantages to washable pads, but one of the advantages of disposable is the infused scent attractant.
