Cry Need advice on liver shunt and kidney stones

Our 4 year old male Choochie, just started having problems peeing, he was straining and obviously in discomfort, he was taken to the vet about 5 times for catheterizing.
He started getting tests and the one vet recommended taking him to a specialist, he was diagnosed with a liver shunt, and his left kidney is almost non functional due to kidney stones.
Right now the options are not good, if we opt for the surgery route there is a 5 - 25% chance that the anesthetic will kill him then if he survives there is the issue with seizures and other complications resulting from hypertension.
Has anyone gone through this? anyone tried a holistic approach with radical diets etc. I keep seeing "milk thistle" mentioned. Are we too late to start trying to detoxify the little guy?
