I didn't read that she has had a thorough exam from the vet...did she? The vet really needs to rule out that there is nothing wrong structurally. Was a stool sample done?

If everything structurally is sound, I'd recommend you switch her food asap. Some dogs' anal gland problems come straight from the fact that the food they're eating is irritating their GI system, resulting in irritated and full anal glands. I know you say you want to stick w/ Nutro, but if another type of food/protein source/carb source etc would solve the problem, it's worth giving up that food.
