First of all, I totally feel your pain. I think you should keep searching and find another vet. This one doesn't seem compassionate or understanding (the least he could do is talk this stuff thru with you and explain why HE thinks something is the right thing to do and not poopoo on your concerns).
I was thinking that maybe you could drizzle a bit of broth or even just water on the kibble to soften it up for Kayla? I totally agree with the heartguard concern if the amount you are told to give is good for a dog up to 30 lbs. That's not right. I give heartguard but I'm pretty sure it's a dose for up to a 10 lbs dog.
