Right now I have the following pages:
*Pet Care for Sitter Info (Vet Contact info, food/treats info, allergy/med info, notes section)
*Feeding Log (date, time, amount, notes)
*Vaccination Record (date, expiration, vaccination, notes columns)
*Health History (with date, diagnosis, meds, notes columns)
*Medication Log (date, med, dosage, interval, vet contact columns)
*Weight Log (date, time, weight, notes)

I will be adding "dividers" that will have a near full page pocket to keep loose papers I may end up with.

What a great idea. You may want to consider a EMR (Emergency Medical Record) like program. It will make changes easier and you will always have a hard copy.
