Going through with the first heat leaves them open for mammary cancer and for the life of me I can not remember the name of that infection unspayed dogs can get that can kill your dog in 24 hours. Also many people are just not responsible enough to have a dog in heat (not meaning the poster) but they think a diaper will keep a male from mating with the female and will let them be outside with out supervision and such.
