I read that if a puppy is taken from his mother too early it can have issues with biting too hard. The book said that at around 7 weeks the puppy starts to play bite the mother and the siblings. When the puppy bites the mother too hard she "puts him in his place." So that's how he learns. Brandy was rescued at 5 weeks, and whenever she bit me hard I would sternly say, "NO." Then she would back off. I haven't had an incident with her yet. But she does growl and nip at anyone who comes near me while I have her on my lap. She gets so mad if I try to pet another dog while I'm holding her.
