I would def. call your vet. We just took our new pup, Rocky, 2.6 pounds off of tresaderm. Tresaderm was prescribed for his ear mites and were administered through ear drops. He became extremely hungry, not playful, moody, and his eyes were glazed over. Took him to the vet and he said he could administer Revolution, and I asked if he would have a reaction to that. He threw his hands up and said little dogs have reactions that bigger dogs don't.
Anyways, I think some little dogs can't handle conventional medicine, and need alternatives. Needless to say we are taking our pup to a holistic vet to seek out alternative treatments. Basically, our one vet didn't know how to help him.
