I'd contact a vet nutritionist (board certified) for advice on feeding. I use Dr. Rebecca Remillard - she's with North Carolina State Vet school now but I've used her for many years and she does phone consults. Feel free to PM me if you need her contact info. As for a regular vet on LI with more experience, personally, if this was my dog, I'd want an internal medicine specialist to be managing the case.
