Sounds like you have a good, conservative vet. It feels like a losing battle sometimes with Yorkie knees. These are dogs that love to run and jump. Trixie comes from reckless runners and jumpers, both sides of her family, so I know how difficult it will be to keep her from jumping. Whatever, proceed slowly and carefully with talk of surgery.
