This sounds like more of a problem than this might help. But try this:
First try NOT to hold onto him tightly. Dogs hate that. If a leash will work do that, but if you must hold him, do it lightly. I know that is hard when they are scared and flailing around.
Also dogs lick and yawn to quiet themselves down. Try mimicing his behavior back to him. Give lots of 'kisses' prior to going to the big bad vet. Then do so while there, or yawn to him. Yes, the vet will think you are crazy, but so what, you love your dog. Dogs learn to hate the vet somewhat because of our react to going there. If we hate it they will too. Show him that you understand his problem and speak his language to calm him. This may or may not work......
