This is not medical advice. This is just what happened to Gracie. When she was about a year old she came into a long heat. (She was a rescue and we had already scheduled for a spay.) That turned into a false preg. (there was no contact with fertile males). Her teats swelled and even leaked fluid. Her vulva stayed puffy. After about a month of nesting behavior with toys, the heat and false preg. ended. Then she was spayed. Even after being spayed, once in awhile she gets into nesting mode and hides her toy babies in her crate, she fusses with them for a few days then stops.
