Try putting the pee pad on a slightly larger square of something like linoleum or a rubber mat for easier clean up of misses.

He might be hitting the same spot over and over again because he is attracted to the scent. Clean up the flooring with an enzyme cleaner to remove the scent.

What kind of pads do you use and what kind of surface are they on? If you are using disposable pads on carpeting for instance, maybe he doesn't like the sound that the pad makes when he steps on it. If the pad is in a holder, that might be encouraging him to aim from afar. I have read instances here where dogs will pee on the side of the holder and think they have hit their mark.

Is there anything else around the pad that maybe makes him stop a few steps short?

Does he always do this or does it occur more frequently as the pad gets fuller? You may try switching the pad more often then.

I hope one of these suggestions help solve the problem.
