I'm really sorry to hear about this - it is always scarey when this happens. It could be something as simple as a bad film or something that has been there before and if they don't have her old films to compare to, then they wouldn't know that. Anyway, the point is that if she has mamograms yearly and by the way you said this was her yearly exam, I think that is the case - even if this turns out to be problematic, with the fantastic medical therapies available, she is almost sure to be just fine.
I know that it is terribly stressful - be strong, especially for her.
