I do it as often as it needs doing, which is different for each dog. Some don't need to have it done, ever. Some need a diet change to help the glands function properly, usually more fiber works wonders. Here's a good video showing how to express the glands by the external method. It can also be done internally, some don't express easily externally.
PS: I don't agee with this person's descriptions towards the end... but her method is right on.
