The extra small Thundershirt fits my 2.15 lb girl almost perfect. It's a little big around the neck, but fits really good around the belly. I called the Thundershirt company directly and spoke with them about sizing and ordered from them. They were very helpful and they have a really good return policy...they donate returned item to shelters and rescue operations. I also bought my extra small console car seat directly from the Snoozer company. I have to purchase most items for my little one online due to her small size.
