We don't adopt out to homes that have intact males or females...part of our policy...unless they have a medical reason (confirmed by vet) that they cannot be. We just had someone write to us about one of our fosters asking if he was neutered or not because she wanted to breed him with her female....
One time we had a female come into rescue that we were told was spayed.... and our vet thought she was too....well turned out she was not she went into heat into her new home, but there was no issue as the other pets in the home were.... so it can happen of course once it was safe we paid to have her spayed.... however we did tell the adoptive family that we had not spayed her but was told she was... but had no proof....
