I think I read one time that even after cleaning the rug the black light still picks up where the spot was. I also heard that steam cleaning somehow seals in the stain. I don't know how true that is but it seems that after all the cleaning the OP has done the carpet should be clean. Having a carpet cleaning machine with good suction is very helpful to pull the urine out of the carpet. Of course it works even better if you get the accident right away.

One thing to be sure to watch for is if you have wood under the carpeting. Urine can seep into the wood under the carpet and that is very difficult to fix. Wood is porous and holds the smell. Sometimes the floor boards have to be taken up in order to be rid of the smell.
