If he has an infection this might be causing him to being lethargic as he doesn't feel well.

A vet visit definitely is in order when you can get him in. Or if he seems to be getting worse the emergency vets are open 24/7.

I agree I think you should go to the vet also. The lethargic part would really scare me.
