He's far from potty-trained and too young to be reliant - could even have a bladder or tummy irritation or anything causing some upset - but in any event all puppies seem to lose their wonderfully early-learned house-training because we start to watch them less, trust them more and they just lapse back into going wherever is convenient and easy. Go back to your original crate-training where he's in the crate when you leave the room or are having dinner, running errands, etc. Otherwise, keep to his schedule and keep him outside the crate, involved in your life but watching him closely for behavior that indicates he's about to go and then rush him to his potty place and give him the command to go potty. But don't feel bad because he relapsed some, all dogs go through times like this - no matter how old they are and how much control they may have when training begins.
