Omgosh...I had exactly the same problem and have posted it on many threads as a warning and was starting to feel like an idiot about it, as no one else seems to have this issue.
ZoE backed out of the Puppia step-in harness in my front yard (I live on a main road) and it was scary as could be. I figured I had to have put it on wrong, so made sure it was secure and carried her to the backyard, where she got out of it again, by pulling against the leash, til the leash was tawt, and then backing up. I watched in horror as once again, the harness slipped over her arms and head and she was running free. I threw every harness of that style right in the garbage!
ZoE now has a Buddy Belt leather harnesses and I love them! She has never gotten out of it!!! I highly recommend it. She's secure and it doesn't go anywhere near her neck.
