I agree you may need an in home visit. We can give general, generic advice but, to really see what is going on and what the triggers are. My self I would treat this as an adult rescue and start training from that point of view. Start a traing plan of basics. Bad habits and behaviors have been going on for a long time. Dogs repeat behaviors that bring them rewards or satisfaction. You have to teach Yorkies how to cope with excitement (people coming in the door) going on walks etc. Many dogs bark when left alone out of boredom, or anxiety. It sounds like your Yorkie has too much freedom and stimulation when no one is home. When no one is home your dog should spend most of his time sleeping not barking. All dogs need exercise and interaction maybe you step that up when you are home. A tired dog is a happy dog. Please read some good training books. I would suggest Dog Dilemmas by Gary R. Sampson DMV with Dick Wolfsie This is very close to my method of : How to train the Just Right Yorkie for You.

and Dog Perfect by Sarah Hodgson
