Medication is not working...2 vets and no one has done an xray. You need to advocate for your baby and tell them what you want done. If you don't want him to be sedated for xrays ask them to try without it, honestly I think you need to find a reputable vet in your area, even an orthopaedic specialist at this point.

This could be an infection, neurological, or a skeletal defect, there is no way of knowing based on what you are describing. I am however, concerned. A 3rd opinnion is not unreasonable at this point, there is something wrong with your baby and you gotta do what you gotta do.
