Are you giving her a treat when she goes outside or on the pad? If yes, up the treat to a piece of chicken or something she loves. Everytime she does it right, say 'Good Girl' and give her the treat. Treats and praise are 'key' in positive reinforcement. Ignore mistakes, clean them up and move on. No grumbling, sideways glances or stinkeye. Just do it.

Also, instead of crate or xpen, you could confine her in one room (puppy proof) until she hones her potty skills.

Also watch her closely, she should be giving you some kind of clue that she's thinking about 'going'.

And when I say 'watch her' I don't mean 'keep an eye on her', I mean look at her every move and figure out what she's doing, going to do, and what she's thinking about.

Pretend she's a poisonous snake that you're trying not to let bite you. Eyes on her. When you need a break, she goes with you or into a crate, xpen, or puppyproof room.

Extra credit: Every time DH wants anything, tell him to wait, you'll be right back, you have to go walk the puppy, she has to go potty. Nothing like a supportive DH.
