My Yorkie was diagnosed with this as well, and we give glucosamine/condroitin in pill form. You can find this in any place that sells humans vitamins, they even have it in powder/liquid form for dogs, but I find its more expensive (but may be worth it- if your baby is picky). I like to buy higher quality, not from walmart. We use it for just overall good joint health, it cant hurt. Call your Vet for correct dosage info.
Just be sure that if you do use it, to be consistant with it, to see the best result.
