Has a biopsy "needle aspiration" been done? I would do this first before anything else, a round of antibiotics probably wouldn't hurt while waiting for the results...
I had a Black Lab that presented with similar symptoms many years ago, we thought the worst - a biopsy revealed that it was nothing more than a benign lump of scar tissue. The vet was able to remove it quite simply with local anesthetic and a mild sedative. Shine came home and live to be 16.9 months old!
