I would go to a different vet, probably an internal medicine specialist and/or a dermatologist. You will very likely save a lot of heartache, and time, and money doing this.

I would also switch foods. Ziwi Peak has one of the highest fat contents of any dog food on the market. The food is concerning not only relative to weight, but relative to all of the other problems your girl is experiencing.
