Default Tilly Scratching Front Left Arm Pit Raw

My 12 year old Yorkie has started scratching her front left arm pit raw. We do not see any other raw rashes but she has been chewing her paws. It has been suggested it could be allergy related and the vet suggested giving her small portions of benadryl. It has also been suggested that a common allergy would be grains in food so we have switch food. But that has not helped yet. If it is a food allergy, maybe we need to be patient.

We are looking for a recommendation on an ointment, salve, or powder to take the discomfort away and stop the scratching. Would a hydrocortisone cream be of any help? Is there any thing that should be avoided?
