Cotton balls won't stay in my boys' ears either, and they are big boys. Some people use foam ear plugs. I have been meaning to try that. My only concern is that they might push hair and dirt further into the ear.
I haven't had any ear problems so far. We use Vet Solutions ear cleaner with a drying agent after baths, when I remember. Any good ear cleaner with a dryer helps.
