I'm in no way an expert, but I'd like to offer a few thoughts. First, I think you are sending some mixed signals by wanting success with the pads, crate, and outside. I'd pick one method and stick with it....I'd stick with the pads since you've already started. I think the key will really be patience and reinforcement (might want to add a special food treat to the praise). Almost all yorkies take a long time to housebreak and yours will probably be especially difficult given his past. He probably isn't doing too great at the moment due to a mixture of his past conditions and slightly mixed messages from you.
Since the crate isn't stopping peeing, I would think about a puppy playpen where you could put a pad in with him (put the pen on some not too valuable flooring).
Finally, just something to think about with sleeping in the bed...supposedly can create some alpha dog issues (which might reinforce to the dog that he can pee anywhere)....so maybe try a dog bed in a playpen?
