It very well could be a parasite like coccidia or giardia. Those do not always show up on a regular fecal. My vet either does a float AND a smear or sends it out to the lab for more extensive testing...just to be sure. It's better to BRING a sample too instead of having them test at the vets office. Get some that has visible blood, that would help. As for the littermate not having it--he MAY have it but just not having signs. Some are hit harder than others by parasites, diseases, etc. He could have swallowed or chewed on something he wasn't supposed to, could be from GI upset (change of food, stress, etc). There are many possibilities. However, with a puppy that young they can become dehydrated or anemic quickly from bloody stool (especially if it is loose or watery-which you didn't mention). I would definitely keep pushing or switch vets until you get an answer.
