Under normal circumstances you would see the vet within the first couple days of getting a dog... If you have had him four weeks and not taken him to see a vet that is not to wise. If he is still a puppy , he needs to have a vet look him over, he may need to be dewormed, need other shots and all this the vet would give you advice on. If he is sleepy and only a puppy , you may try hand feeding him and or giving him chicken and perhaps some nutrical...If I were you I would get him to the vet sooner then later!!! All puppies/dogs need a series of shots to maintain health.
