Honestly I would not let this vet touch my dog. He should not tell you he is worried to do a simple spay because of her size. Many vet's i've worked for actually prefer the tiny ones because its quicker and easier to do. I would not allow my dog to have surgery without an injectable sedative, intubation, gas anesthesia (iso or sevo), and IV fluids at least. I'd also have a full blood panel done before hand to make sure everything is in working order.

Is this the same vet that jumped into exploratory surgery on Mack and then diagnosed him with parvo? Its just not right. I would be wary.
