I am very sorry for your loss. It is impossible to say what was wrong without performing an animal autopsy. Could have been a liver shunt, neurological problem, or many other things. Falling off the bed and couch may have injured him and added to his problems. A puppy, particularly one with problems, should be confined to a safe area, like a pen or crate.
