It is possible to only have one descended testicle. If I am not mistaken, an undescended testicle must be taken care of.
Is the knot in that area? Or is it a bump you saw to the side of the base of the penis? There are glands in that area that will pop out like a little marble on each side.
It's hard to say without knowing where exactly the knot is.
