If her teeth are rotting and loose, she needs more dental work. Small dogs are prone to tartar, and some are genetically prone to have really bad dental problems requiring dental work every 6 months to a year. It is not unusual to have a cleaning once a year.

Her breath will not improve until the rotting teeth are extracted.

I would be very concerned about feeding a vegetarian diet based on a vet's hunch about digestion. If it were me, I would be looking for another vet and I would consult a canine dental specialist.
