Hmmmmm I guess I am the only one that doesnt stand over their dog 24/7 to watch them poop and pee. I am always at home, and my baby doesnt prevent me from spending time with my dog. The dog is in my room in his gated area. I am not gonna stand there and wait for him to go so I can give him a treat! I give him plenty of time and attention. And his "punishment" is being crated which most of you do. It is amazing how "perfect" you all are! I dont see anything I have done wrong. I play with him, fresh food and water, fresh peepads. When he does have an accident I clean it up with vinegar and tell him no and just lately I started crating him for this. He was never this bad, just lately. I have tried the treat method, spraying him if I catch him and the firm no crap. There is a reason why nothing is working. But I have done almost everything you "perfect" people have suggested.
